Healthy Aging Nursing Coordinator

The Healthy Aging, Nurse Coordinator will provide direct services, coordination of health education programing and promotion of healthy aging to East Providence, older adults Position is located at the East Providence Senior Center, 610 Waterman Ave., East Providence.
Healthy Aging, Nurse Coordinator is a part-time position, (20) hours per week (hours may vary per department needs) under the direct supervision of the Director of Senior Services.

Essential Duties and Responsibilities:
• Provide direct nursing and coordinate all health education programs including Diabetes Outpatient Education. Refer when appropriate to other disciplines such as registered dietician and pharmacist, maintain records, and perform billing for reimbursable services.
• Maintain standards as per the Rhode Island Department of Health and American Association of Diabetic Educators.
• Coordinate health screenings and annual influenza clinics. Nurse may perform screenings or coordinate with outside agencies.
• A first responder to health emergencies at the Senior Center and for any city health issues or outbreaks affecting public health.
• Coordinate all health presentations, usually one per month on a variety of topics. Nurse may give the presentations or coordinate with outside agencies.
• Facilitate as needed or coordinate health related support groups and programs offered at the Senior Center.
• Act as health information liaison for city residents, responding to inquiries regarding health and wellness information, infectious disease, and referrals to health care providers.
• Duties include providing visits to home bound elderly as needed.
• Manage, receive and store all protected health information coming into the Senior Center.
• Recruit, train, and manage health office volunteers.
• Precept/mentor students in their clinical rotations. Student rotations can include: nursing, pharmacy, nursing assistant, and occupational health students.
• Survey seniors and search for other health services to bring to the Senior Center that promote healthy aging.
• Work as a team member with Senior Center staff and city employees to provide positive customer service to older adults and their caregivers in East Providence and others who live in surrounding cities and towns who are members of the East Providence Senior Center.
• Participate in health related committees as requested by director.
• Other duties as assigned.

Education and/or Experience:

Bachelor’s Degree (BS) from a four-year College or University. Registered nurse, licensed to practice in Rhode Island. Certified Diabetes Educator CDE) or a Rhode Island Certified Diabetes Outpatient Educator (CDOE) preferred. Experience in the field of aging and the health related challenges faced by older adult populations. Must have a working knowledge of computer and Internet communication.
