City of East Providence Mobile Food Establishment Requirement's (without State Mobile Food Establishment Registration)
- Completed Municipal Application
- Mobile Food Establishment Fire Inspection Certificate
- Mobile Food Establishment License from the Department of Health
- State of Rhode Island Sales Tax Permit
- Valid motor vehicle registration
- Valid motor vehicle insurance
- Copy of Driver's License
